UE-1:Request for Proposed Construction    With this form you will be able to retrieve the necessary form which needs to be completed in order to proceed with a construction project within your department.
UE-1 Supplement:   Reactivating a UE-1 on Hold.
Underground Dig Permit   This permit will allow the Requestor to do digging, trenching, tunneling or other excavation work that is required, on university property, in order to accomplish the purpose or need identified
Supplement to Dig Permit

  This inspection form verifies the location of current underground utilities such as irrigation, plumbing, electrical, and fiber optics, by the respective utilities companies prior to trenching, digging, and construction for permit approval. Thanks David 
Key Request Form   Print out and fill in the Key Request Form with all appropriate information and signatures. Original signatures are needed to request building keys so you will need to hand deliver or mail the form to the Physical Plant at MSC142. No fax or email returns accepted.
Lost Key Reimbursement Form    Print out and fill in the Lost Key Reimbursement Form with all appropriate information and signatures to include narrative of loss and attempts for recovery, signed by the Dean/Administrator, and police report. The Original signatures are needed to request Lost Key Reimbursement so you will need to hand deliver or mail the form to the Physical Plant at MSC142. No fax or email returns accepted.
Vehicle Request Form and Rental Rates Worksheet     Print out and fill in the Vehicle Request Form with all appropriate information and signatures. When done please fax to (361) 593-2669. Original form with original signatures must be submitted to the Physical Plant Office (MSC 142) on or before the departure date. The Rental Rates Worksheet is for your convenience only. Use the worksheet to calculate your Rental Rates, but submitting it is not required.
Physical Plant Customer Survey   Print out and fill in the Physical Plant Customer Survey with all the requested information. Please mail the completed survey to the Physical Plant Office at MSC 142 or fax to 361-593-2669.
